New Student Information Form
For admission to Southwestern Illinois College, fill out a New Student Information Form on eSTORM or request a hard copy by calling 618-235-2700, ext. 5465 or by visiting one of the campuses. Allied Health Applications 
The 2008 Allied Health Applications are now available. To access the appropriate application, please click on your area of interest.
Note: Please forward the completed appplication as indicated below.
- Students interested in the Medical Assistant program should consult with the program coordinator before submitting our New Student Information Form.
- Students interested in the Massage Therapy program should submit a New Student Information Form on eSTORM.
Allied Health programs' planning guides
Applicants for Allied Health programs are selected based on a point system. These planning guides will help you determine the number of points you have accumulated toward acceptance and what you can do to increase your points.
