You may, at your discretion, grant Southwestern Illinois College permission to release information about your student records to individuals or agencies by submitting a completed Authorization to Release Student Information form.  

Open the Authorization to Release Student Information form. Print it, complete all the necessary fields, sign it, then send it to Enrollment Services.

If you need to request an official transcript, please log in to eSTORM, select Student Center and choose Official Transcript from the dropdown box under Academics or fill out and submit the Official Transcript Request Form to the mailing address or fax number found at the top right corner of the form.
