Please print the form, fill it out and send it to the address listed on the form. Don't forget to mark Programs and Services for Older Persons in the "Apply my gift to" section and list the program you would like your donation to support.

• If this is your first online donation to SWIC or PSOP, you will need to register. If you have previously registered, you may use the login in the upper right of the screen.
• After you click the Continue button or log in, two more fields will appear in the registration section—Amount and Designation.
• Click inside the blank line for Designation to select "Programs & Services for Older Persons."
• To designate your donation for specific PSOP programs as listed above, list them in the Comments field at the end of the payment information section.
• If you would like to designate your gift in memory or honor of someone special, please list this information in the Comments section.
